What's the best method to get the e brake assembly out? Do you have to take out the cowl fresh air vent first?

Yep you remove fresh air box and kick panel. Next disconnect the cable and light switch wire and your good to go.
You may have to take the whole assembly loose to gain access to the cable connector.

No you do not have to take the fresh air vent out. Two nuts in the inner fender area can be a pain due to undercoating. Just make sure to wires brush around them to clean some of the threads before removing. You can brake a stud that way. In the car there is one bolt on the side and the cable has a pin with a lock ring. Remove all those and pull it out...

Sure it can be done with the fresh air box in but save yourself the trouble and remove it

Which holes do you mean? The small holes in the firewall are for the plastic nails that hold the insulation. The 3 medium size holes are for the studs on your heater box. The 2 larger holes are for your heater core....

Is there a method to setting the tightness of the cable itself. I know there is a way to adjust where located where the cable from the pedal meets the cables going to the rear, but is it just a guessing game? Or do I just leave that alone?

Make sure pedal is in full up position and tension the cable at the splitter you can tell when it starts to move the shoes if you jack up the rear and spin the tires.
