i am also looking for the panel that goes in place of the dash push trans panel on left side of steering wheel
 
Welcome from North Carolina. To answer your question, NO, Dodge never had a Hurst equipped 64 Dodge. If I recall, the 1970 300 is the only Hurst Mopar.
