Never put discs on the rear of any of my cars. But if I did, I would get ahold of Cass aka Dr. Diff. He sells some nice stuff and knows what works with what.
Dr diff has two sizes of rear discs. Master power brakes has a kit. These will work with stock bearings. The ram man has a kit which I believe will work.
If you want to stay all Mopar, go with the latest Mopar Action Magazines, I believe it was during this last summer, the complete rears off of a Jeep Liberty. Uses a thicker rotor. larger pad and inside drum brakes for the e-brakes.
I'm liking the looks of the Dr. Diff stuff so far. I want over the counter wear parts because my daughter intends to drive the wheels off her charger. Now where is that 5 spd?
The liberty swap does not require green bearings, if you follow the mopar action article. Ehrenberg is a fanatic when it comes to the stock bearing setup. It is a lot of work though.
Eberg has tried to talk me into the Liberty swap a couple of times but I don't see any value in it when the Doctor Diff kit fits and works. I've used the Doctor Diff stuff on several different cars and it is a fairly simple bolt on. Biggest hassle is redoing the brake lines and sorting out the e brake cables but you're going to have that work with any rear disc kit.
