Which is driver and which is passenger side? Do they install over or under the rear panels. (The panels that the window crank and armrests go through)

Top one is the left, bottom one is the right. The outer raised ridge will show outside the rear panel. The front forked end goes under the sill plate.
 
I’d say the bottom is the left (drivers) side. They go on after the door panels are installed so you see the whole piece of trim. The fork goes under the sill plate.

You can prove this to yourself. If you put a screw in the hole of the extension piece it sits at an angle. The hole in the top of the rocker area is inboard, not straight down. Just like the end screws in the sill plates.

I, after looking at a parts car that the sill plate extensions are not removed, agree with you. I was wrong on the left and right.
