No, I use 2 of the Moog offset bushings on each side of the car. One pointed out and the other in. In the Moog instructions, they say to only use one per side, I believe the rear.

Ah gotcha... I must have been having a "senior" moment.. makes a lot more sense now (actually I always thought that you uses 2 per side...).
 
If your ride height is low you will not get the positive caster you desire
You have it backwards.
As the suspension compresses, the caster increases. A lowered car has more caster, a raised car has less. The steering knuckle/spindle and UCA arrangement moves the upper ball joint rearward under compression.
 
Depends on the frame angle. If the front is low and the rear is high you will have a hard time achieving the positive caster you desired
