I believe I found the real issue to my problems. My fork pivot bracket is bent so that the fork sits to far out. I think that the bracket should me about 90˚ , mine is not!

You are probably right, the fork slipped off the bearing again, looking for the right parts now to make this work. Fork mount is stamped ”A-body”
