Here's a long shot - get a hold of Bishko Books and see if they have a factory upholstery book for 2004-2006. Books with low demand are sometimes cheap.
Best I can do with an assembled car Don. Rule is shoved hard against the carpet. My car is A/C, so the box pushes the folding rule down a bit. I can feel the firewall but it won't flex enough to make contact.
Headrests were an option until Jan 1 1969, and the back looks like 69 also. They were recovered at some point, of course. Looks like it may have been a shop that reused the covers and replaced the worn centers. That used to be common in the upholstery business.
Take a seam ripper and cut open the end where the webbing is sewn together around the buckle. Maybe a rear one would fair better. Fair chance it will be less sun-faded. Google up "four-panel 2" webbing. At least i think it's 2".
No one will know, because there is a good bit of variation Depending on plant of the build, date and even the shift, all can trigger color or part changes - painted buckles vs chrome, contrast or matching webbing. I wouldn't sweat it a bunch.
If the relay inside works, you're halfway to getting it fixed yourself. Cleaning it carefully and adding neetsfoot oil to the jewels with a toothpick gave me a clock that keeps time in my Charger. But, as Kiwi alluded, there are some metal tabs and odd fasteners inside that are unforgiving.
I recall reading about the heel pad in one of the magazines a few years back. Maybe check the box date. They should make it right if you didn't trim or cut the carpet yet.

The factory carpet on my console was notched for the side trim clips. Trim goes on after the carpet for sure. Get a good glue - 3M Super 77, Armaflex, or similar.
Sounds like the regulator. Factory service manual will be a big help. Reproduction glass channel run weatherstripping that is the wrong dimension ( too thick ) can stress the regulator.
