I had the same problem. I tried drilling a small hole in the end of mine to lube the bushing, didn't work. Then I noticed the bushing was actually spinning in the housing. I ended up getting a new motor.
This isn't the greatest picture, but you can see that the full floors including the gas tank were completely covered. When I installed a new tank, I undercoated it as well.

I have owned two different factory air conditioned cars, a 69 Barracuda and my 66 Charger. Both came with full undercoating. Lately I have noticed a few ads for cars with factory air and they also had full undercoating. Were these two options tied together or is it just a coincidence?
Not sure what you are working on but on my 66 Charger the grommet is right below the voltage regulator, and the line picks up vacuum from the power brake port in the intake manifold.
