Im not sure if this is the right jack for my 65 belv 2. it was in my garage, not sure if i removed it from the car years ago. it fits the bumper pretty good. I just need someone else to tell me it is

I have this jack. Came from a 63 plymouth. But is it correct?
All the comparable jacks on ebay with the side notch mast locator are chevy

Nick's Garage - Jack/Jack Hook Identification
 
I have studied Nick's garage i.d. page numorous times. No detailed jack base shots.
My jack and 65belvedere have the same mast head, and base.
The up/down selector on BigFlo's is on the opposite side as ours.
My jack hook has no part number, so it can't be mopar.
Rivit on the hook comes in on opposite side as a mopar hook.
I'm going to say we both got GM jacks unless 65belvedere's hook has a part number.
So mine is more scrap. BUT IT CAME OUT OF A MOPAR!!!!
 
My 65 Belvedere has been in the family since new and I posted pics above, of the original jack and hook. Nick’s Garage info contradicts my jack. His pictures of a 64 hook are what I have for my 65. I’ll need to clean my hook to see if I can find a part number - right now I don’t see one.

I don't doubt what you have there but doing some searching it appears all mopar jacks have the receiver slot as part of the jack, and not spot welded to the jack like yours. A lot could be what plant the car came from and the jack vendor that they were using the time.
