hard to tell..... exhaust leak driver's side? Have you had this engine apart recently? Maybe try a mechanic's stethascope or a piece of pipe pressed to your ear to narrow it down. Could be as simple as a slightly bent pushrod or worn rocker arm on one valve.
If it's an exhaust leak (Which it sounds like it is) a length of hose is better than a stick... Or a long screwdriver... If using a stethoscope try pulling the probe & diaphragm off so the hose is exposed....
