have a new ratio tag to install on a recently rebuilt 489 case rearend...which is the correct stud to mount tag...thanks in advance Rob.............
If a position then you have two tags...the ratio and limited slip tag....
I will post a pic later...
Thanks....one less thing I have to do tonight, lol...Ratio ID tag, driver side middle bolt. Suregrip tag, passenger side middle bolt
Here's a photo of my 1969 '489 case showing the RATIO tag. They used a orange paint splotch just below the fill plug on the other side to indicate a Sure Grip. No tag for that, at least in '69 that I'm aware of.

All I know is 68s had a round disk. Pretty sure it continued through 70
Mike,You've got valid points about his avatar car and his lack of information on what car he's working on. BUT, he did say a '489 case and I don't think those came out until late '68 (for '69 MY cars) IIRC?
So, the '489 must not be original to the '64 Polara in his avatar.
As far as all the "paint stripes" and "colors" you mentioned, I agree with all that. I also have two '69's with a '489 case and neither have a Sure Grip tag, just orange paint.
Some info from "Moparts" below. I know I've seen the "orange" splotch talked about on other sites too. So maybe it all depended on which plant the car was built at whether it was an orange splotch or a tag?? Mine was "orange", I've owned the car for 44 years and I'm the only one to have ever done anything to the rear end. That's not to say other '489's were not identified differently, ie., a tag like you say. That's the beauty of Mopar, nothing is an absolute with these cars!
From Moparts:
After 1965, a tag was affixed under one of the
carrier mounting nuts to identify the ratio. If Sure Grip equipped,
an additional Sure Grip lube tag was sometimes affixed; later years
sometimes had the filler plug painted orange.
