Anybody good at identifying disc conversions?

Might anyone know what caliper this is? or what it is off of? It's on a 69 RoadRunner and the guy I bought it from said it had a "later disc conversion done to it." Shame on me for not getting more info because now I have no clue. I've looked on some of the links in other threads but it's difficult to back trace. The yellow writing on the parts are 7/25/95 (I need to double check). Is there that late model of a brake that would work on these? Even if I can't figure out what they are, I really need brake pads. Another strange thing is the bleeder screws bottom out in their holes and won't hold fluid. I went to the parts store and they didn't "have" one long enough. The one's they had matched what I already have. Whoever in the past had stuffed the bleeder with an epoxy of something just so they'd hold pressure - real nice.
It's an 11" rotor. It's definitely a Chrysler product. The numbers on the caliper are below.

The upper center emboss has 4542B
Lower left numbers are 3688
Lower right is Pentastar 882
Any help would be greatly appreciated! Let me know if I can get any more info or pic's.

Can you give me a pic as to how the caliper mounts to the bracket? That might help me some. I work is a parts store that's in the parts warehouse so I can look at different things. Bruce
I just tolked to a buddy of mine and he checked out my '77 Cordoba and I think that's what you have. Let me know if this helps you out. Bruce
 
It's a slider caliper, rotor size is 10.87 or 11.75 (not sure without measuring). Any parts store will have the caliper (ask for a 73-76 dart/duster).

If that's the general assumption I'm definitely willing to try it out.
 
I will also add, one of the bolts that hold the caliper in place is slightly longer then the other on both sides. I'm not sure if this is a distinguishing factor or if the guy just ran out of bolts haha
