De-code my card from Chrysler Historical

Can anyone help me de-code this ticket from Chrysler Historical? It's for my 67 Coronet 440.

Those things are blurry as hell a lot of the time. Mine was way worse than that one. If you find a picture of a non blurry one on the internet it will help with figuring out the wording in the columns.
 
I can do that in 20 mins, just don't have 20 mins anytime soon.
They're pretty simple, columns 1-80, each punch represents a string, like the vin or a date, or a 3 digit code, check this:
 
You also need a book that has the codes listed such as galen's white books and others. The IBM card will give you the codes, but if you don't know what they mean it doesn't help much.
