Used, all threads are good, ports are unmodified. Top near one bolt hole is worn. There is a small circle void in the center of the plenum (see last picture), doesn't lead to anywhere, leave as is or thread in a pipe plug.

It does lead into the heat crossover doesn't it?..
You would need to plug it, or run on heads with no heat crossover.
Otherwise it could affect the performance...

No, it does not lead into heat crossover.
I filled it with water and did not leak out anywhere.

EGR hole.
Where the EGR would sit on the outside of the manifold, it is plugged. Hence, leads to no where.
