Just as a note of interest, the jacks and bases were not brand specific and were supplied to GM Ford and Chrysler by outside jobbers.
The Hooks of course were brand specific.
Here are some pics of the base Dan is after:

I have one from a '68, not sure if it will work for you (note the 'T'-shaped top slot)...if you want it just PM me and you can have it fer nuthin'.
