The Malwood pedal with the master mounted under the dash is a pretty slick piece.
With a manual transmission, you have a lower heavy steel 5 bolt cover, and there is also a small shield that goes behind the starter. Nothing else. Those 2 parts are different between 10.5"/130 and 11.0"/143 bellhousing/flywheel combinations.
What I noticed from watching your video showing the linkage is a couple things. Yes, the nuts were loose...but look how much slop is in the lever and the shaft. With the nuts loose, the levers should fit the flats on the shafts well. I would say your shafts are worn out. So even if you...
Here are my notes on the LK700 bellhousing when they first came out...not sure if ANY of the issues have been fixed or not. The problem with the pivot is that the holes are not drilled/tapped in the correct location.

It looks to me like the where the flywheel bolts to crank it is counter bored?? If yes, that flywheel is definitely the problem. You could have someone cut some off the engine side to gain clearance, but then it would need re-balanced...and I would question ring gear/starter location.
