I think your problem is the Hemi never originally used the 143-tooth flywheel, only a 172-tooth from '66-'69 which placed the direct-drive starter further outboard, away from the manifold or a 130-tooth in '70-'71. Later manifolds might be different.

The original side outlets (factory called them spot coolers) were chromed pot metal mounted on the kick panels fed by flex ducts, I doubt if anybody is re-popping them. If you can't find used originals, you could probably find universal aftermarket registers that take the same size ducts and...
The factory 143-tooth is a 6-bolt Wedge flywheel although it is available in the aftermarket in the Hemi 8-bolt configuration. The '66-'69 Hemi wheel was 172-tooth.
The '70-'71 Hemi used the 130-tooth and same aluminum housing and starter as the Wedge.
