Grommet for AC heater control and vacuum line

Looking for a replacement firewall grommet of the same part number or something that will work in its place. Original grommet part number is 3431279. Vacuum line from intake manifold to the A/C control and wire line from A/C control to Hot/Cold valve pass through it. Anyone have a source or know something that can be substituted. Picture is from engine side of firewall.

You can probably buy a grommet at the hardware store but, certainly not a dual sleeve like you show. Search the Mopar vendors on this site or just Google them.

Look at DMT #CHR2889809 its for the foot operated windshield washer hoses.
