Trying to figure out what happened. Only 300 miles on this motor. Out it comes.

Roller lifter failure? Looks like it bound up the pushrod and ended up snaping the rocker under stress.....Pushrod should have bent before that though. Wow that sucks man. Hope you figure out what happened man and get it back in action quick! Best of luck to ya.
 
Pushrods WAY,WAY too short. Bent or broken pushrod, breaks rocker, chucks out roller lifter, other roller goes sideways, chews camshaft up. Ask me how i know.
Edit. You have about five threads showing under your crane rockers. Correct is zero to one. More threads, more bad sideload leverage on the adjuster/pushrod tip. I had the exact same result with mine,also with cranes. .200 longer pushrods solved my problem.
Had to regrind my roller. Slowed down three tenths.

First I thought the cam was wiped but the lobes I see look to be okay. Maybe one of the rockers gave up first then it just dominoes from there. Where did the push rod end up? What spring rate were you running?
 
The free lifter slides up and down in the bore like butter (not sure about the other one yet). The cam looks to be ok. The roller tip on the free lifter has no gouges. I have no idea where the broken piece of lifter went. I’m betting windage tray or bottom of pan. I thought about just fixing what was broken on top and running it but for piece of mind I will just pull the motor and go through it.

This is correct. Some folks think "aw it's just imperfect geometry", but the reality is, you can see the incredible stress you are putting on the rocker with pushrods that much too short. The rocker has no strength with the tip of the adjuster cantilevered that far from it's body.
 
I noticed that the exhaust rocker that broke has one more thread showing than the intake, which could mean the exhaust was adjusted too tight to start with. Don't know how close to coil bind or other clearances to know if valve train ran out of clearance. If not coil bind, could be Piston to valve, or even retainer to valve guide? As mentioned, the pushrods were too short to begin with.
 
Or the rocker just couldn't take the stress of too much spring pressure and split in half. Then the push-rod fell out and on the next cycle got bound up under the rocker shaft destroying the lifter and carnage from there. I split an INDY Black rocker this way a few months ago. Rocker just couldn't handle #650 over the nose. But yea looks like the push-rods were to short to begin with.

Non stock parts was the culprit.

non stock parts (aftermarket non OEM) not installed/used properly
or possibly not checked properly
or parts failure due to the above conditions
or parts failure period
all contributing to the parts failure
more like

( #1 like too short of a pushrod, or #1a (?) improper adjustment
putting the pushrod tip in an over-center position
it's plausible may have shifted in/on the lifter, #2 breaking the aligning strap
#3 the adj. sticking out, it's starving or creating a situation where less oil
in the cup/pushrod tip than what it should have too far away
& bad leverage/fulcrum points
#3a because the rocker adjuster/ball 'is sticking out way to far'
part is prone to failures, I'm surprised it didn't break the adjuster 1st
before it broke the rocker or broke the roller lifter's alignment bar
#4 all the above contributing or creating a problem/s

Correct me if im wrong, aren't Indy rockers T&D?? Or not any more?

I don't think they are T&D. Several reports of the Indy rockers breaking.

I think the intake pushrod is bent too?
Maybe the exhaust pushrod bent, and fell out of the adjuster, then beat the rocker to death.
Those Rockers look like Harlan Sharpe? They are pretty good, but I don't know the lift and spring pressure used here? I mean the rocker could have cracked? sounds like not too may cycles on them?

Also, what type of rocker side clearance?
 
I was thinking the roller rockers & adjt. look more like
Crane Super Gold's, offset style

HS would have needle bearings on the shafts
 
Are those 1.5 or 1.6 rockers? Spring bind at full lift? Hydraulic roller or solid? I agree on the pushrods being too short.

Absolutely. We just saw another member here have the same problem, they also had the adjuster cranked way down...and as (I think it was 'Lewtot') pointed out the adjusters in these rockers nowadays are made longer than they used to be = even less threads should be exposed on the south side of the rocker.
