Starter motor issue.

Hi,
I'm wondering if anyone can help.
I have a 440 big block with a 727 transmission.
My starter motor keeps twisting out of place. I have a high-torque mopar starter motor, it manages to twist itself enough so that I cant start it after. This happens every 10 or so start ups. Bolts are tight with lock tight but still manages to twist.
Has this happened to anyone else and if so what is the answer to fix this?

Seems to me a 5/8" wrench fits both starter cap screw and nut on stud? That would make them both 7/16". Am I having a senior moment?

Does the starter have a snout, that fits into a recess in the transmission?
Yes, it does. It's not snug in the hole it is slightly loose. I spoke to a mechanic friend of mine, and he said there might be some kind of sleeve that goes into the transmission recess to make it tighter on the snout?
 
The starter nose going into the recess in the bellhousing is the key to alignment. Plus, say a Denso Dakota starter , one hole is large, larger than 7/16". It should not be needed, but I made a bushing on my lathe to correct that large hole in 2 of my starters. Never had a problem or failure without the bushing.
