Need help getting back on track getting fender tag replaced

Hi everyone I am restoring a 67 L code 440 4 speed and I had all the information needed to get the computer card for my car and a person who could make the tag but unfortunately I had some major heart problems. And over the months I have lost a lot of information as to how to go about this. If anyone could help me get back on track I would greatly appreciate that. I am doing fine now so I am getting back on track.
