Grooves worn in mis-installed balancer. Replace?

should I replace this thing, or is it probably ok? And does this sort of wear bother anyone, to where I should start looking at other parts as a problem?

No, I have never had a problem with any I used from napa. Myself I put a wipe of gasket seal on when sliding it on a yoke or balancer. New seal in the cover and a little lube and install.
If you do go this route, when you go to the parts store look for a counter guy with grey hair.
 
Did a speedy sleeve from NAPA on my yoke. Had to put a block of wood on install was hitting pretty hard but when on.

They recommend a little sealer (silicone sealer) on shaft to make sure no leaks.
 
The groove is easy to take care of with the sleeve. However if the elastomer between the inner hub and outer ring is deteriorating, its time for a new one.

That’s exactly what my uncle told me about getting my car aligned, if there’s nobody with grey hair you’re at the wrong place lol.

That wear isn't due to mis-alignment. It is common on original high mileage motors, although yours does seem a bit severe. You can sleeve it, or just replace the whole balancer for a new one. There are new ones that look identical to the original for under $150.

One thing to keep in mind, that wear is due to the timing cover seal. It is very tight due to its metal band that keeps spring pressure on the seal itself. That also wears out over time, so I would inspect that now while you have the balancer out.
